How it happens
A team adopts an AI tool or builds an AI feature under normal delivery pressure, without a corresponding investment in the monitoring tooling, data quality processes, or staff training the system actually needs to run safely.
Why it matters
A system that's under-resourced to operate safely tends to fail quietly, through neglected monitoring or an untrained operator missing a warning sign, rather than obviously.
